MSTS is really the only thing out there now. There are 2 new efforts underway, RailDriver's TMTS and a new simulator from KUJU, the original developers of MSTS, but neither is out yet. The forum on UKTrainsim.com has status reports on both.

As John has said there are several good N&W routes available that let you run from Walton to Cedar Bluff on the CV or Williamson, Wilcoe, Weller, or Gilbert on the Pokey in addition to crossing the Dry Fork. Also all 3 routes he mentioned have excellant track diagrams available at train-sim.com to guide you.

There is also a P&WV route set in the N&W era.

Using the RailDriver Dash 9 console makes operating a train much easier than using the keyboard or mouse.
