On B&O's N-1 Duplex 4-4-4-4, one set of drivers had to have the tires turned to a slightly smaller diameter so the two engines wouldn't get in step and stay there.
If you listen to recordings of the 1218, you'll hear the two engines getting in and out of step. The A didn't tend to get into step and stay there . . .

Don't know about the general case, but it may be true of any steam loco with four cylinders and two sets of drivers. One consultant on the design of PRR's T-1 (4-4-4-4) suggested having different driver diameters for each set to minimize the possibility. Maybe it's more likely to occur with larger driver diameter ? Or lower factor of adhesion ?
