Yes, very sickening. Should be the responsibility of the maintenance   supervisor or leader to make sure the switch is lined and locked so no equipment can enter that track.  It is responsibility of engineer to stop short of equipment on track ahead or misaligned switch.
  Lots of little mistakes leads to accident.  Glad o one was hurt except engineers pride. K. Borg
