[StBernard] Consent for Hazard Removal form

I can't locate the form for "Consent for Hazard Removal"   on the sbpg.net
website .  I saw the Demolition form, and, the debris removal (right of
entry ) form, under the "Rebuilding Resources" listing,   Is it listed in
another area?  I'd like to download it, as I know several people who have
not had their homes cleaned/gutted yet, and, are probably not aware of this
project.
