When I signed up by sending by mail 3 or 4 months ago, I needed to send in a
copy of my driver's license to the parish to do likewise. This way, it will
help identify the owner (and address) of the establishment being gutted.

Unfortunately, it is taking an extremely long time for them to get around to
folks. We just hope that there will still be "angels" who are going to be
around by the time they get to ours (at the cost "pros" are charging to gut
houses.
