   Yes, get it notorized. I got mine back in 8 weeks, but I also filled it
out back in December. Boyfriend did his in March and hasn't had a return
yet. That's about all I can tell you.

Someone said you could fill out this form yourself. I have the forms and
they don't seem that difficult to fill out. But there is a place at the
bottom for a Notary's signature. So, evidently, the form must be notarized.
True or not?
