Dear Everyone,

   I called the LRA and they said the original application that you filed
was for them to just get a count.  They have mailed an application out
Monday if you registered.  However, it would be much quicker if you fill out
the application on line.  You will need your insurance information to fill
out.  Policy number, limits, amount claimed amount paid.
