The key date is this Friday, December 8.  If you have opted out or settled
with Murphy this is your cut-off date for rejoining the class.  The form can
be filled out and faxed or emailed to both Sidney and Murphy.

if you are in the buy-out zone.  You can always just play it safe, take the
few minutes to download and fill out the form and send it in.  Let Sidney
tell you that you can't opt in.  At a minimum, you will meet the cut-off
date so that can't tell you yes, you can opt-in, but you took too long and
now you can't opt-in.

You have until the end of January to send in the Proof of Claim form, which
has to be notarized.
