anchor names from headers (Setext and atx)?

The anchor is a back-reference to the table of contents. (Note
that it is produced only if '--toc' is specified on the command
line.) And you certainly can link to `<h2 id="myheader">` using `<a
href="#myheader">my header</a>`...
