Answer:  By steam from the engine passing through coils provided for the purpose.

Answer:  None.  Covered by the safety rule that states "thou shalt be careful".

Answer:  Yes, taking care not to fall into the stoker trough - see the above safety rule.

Answer:  Sorry, I can't help you there.

Answer:  No specific knowledge, but it undoubtedly did happen.
