If your doctor used Louisiana Links program then you can get your records
from the Dept of Health and Hospital. If he did not use Links they are not
going to have a copy of your shot record. Links was created for this reason
but not all doctors used this prgaram.
I worked at the health unit and we used to have to fax shot records to kid
doctors all the time.
