We received the same phone call, but we haven't received the form that
was promised.  In the phone conversation, we were also asked if we
intended to rebuild and if we had insurance coverage and how much had we
received from insurance. If we ever receive the form, we will have an
attorney review it and advise us before we sign and return it to
Crawford.  Can we really trust them to be honest about the testing?
