I'll let Lance give you the official advise, but yes others who have made
claims are getting the same survey questions and permission request.  I
believe it is part of Murphy's assessment process.  Lance can tell you for
sure, but I believe that all the information they gather is part of
discovery so that you or your representative can access it.
