Ref: Question - Opt Out Murphy
 
Murphy has an office set up next to the refinery on St. Berhard Hwy.  You
will be given a 11 page document that to read and initial that was issued by
Judge Fallon 2/6/06. There is also a "Opt Out" form that you must sign that
will be faxed to both Sidney Torres and the attorney for Murphy including
another form titled "Authorization to Fax"
 
There seems to be some disconnect as to whether they will provide the form
to you if you have not previously met with them or not. Won't hurt to go
there and ask.
