I've got a copy form SF95, standard claim form for claiming damages against
the feds.

If we can come up with a consensus on how to fill it out, I'll post a PDF of
the form with everything except names and addresses to make it easier.

Two questions that need to be answered:

8. Basis of claim. (State in detail the known facts and circumstances
attending the damage, injury, or death, identifying persons and property
involved , the place of occurrence and cause thereof).
