It took a little internet surfing, but for those who are not required to
file state income tax, and this will apply to many elderly people, they can
send the LA Dept of Revenue Form R-540INS and a copy of their insurance
statement showing how much they paid in 2006 for the surcharge to receive
their refund. Go to
http://www.revenue.louisiana.gov/forms/taxforms/INS-SUP(1_07)F.pdf   to
print this form.  Only list the LA Citizens Fair Plan surcharge, not your
insurance premium. If you have two or three surcharges, add them all
together for a total surcharge amount. ddk
