If you want a copy of a baptism record I need:
(more name than just ' Donna')
 
1. YOUR PRESENT MAILING ADDRESS 
2. The NAME of the baptized
3. The PLACE where the baptism took place
4. The approximate date of the baptism ( as close as you can remember)
5. Your email or phone number helps if I have trouble finding any of the
above.
